DO I QUALIFY FOR MOBILE PRIMARY CARE?

We accept most major insurances!

I CALLED AND WAS TOLD THAT I QUALIFY FOR IN-HOME PRIMARY CARE. WHAT'S NEXT?

We will coordinate a time with you to be seen for your initial visit with a provider.  We may be mobile, but we will still need you to fill out a little paperwork like any doctor's office.  Also, expect to go over your medical history and current medications.

 

Like all our visits, this initial visit will be thorough!

HOW OFTEN WILL I BE SEEN BY MY PROVIDER?

We schedule Primary Care visits once monthly.  You also have the option to be seen at later intervals if so desired (i.e. every other month or every 3 months).

 

If you have a need to be seen sooner, we will do our best to coordinate a time for you to be seen as often as possible.

I AM THE RESPONSIBLE PARTY FOR AN INDIVIDUAL, AND I WOULD LIKE TO HAVE THEM ENROLLED INTO IN-HOME PRIMARY CARE.

SR Medical specializes in treating home-bound individuals, many of whom may have a mental and physical diagnosis. If you are the responsible party for an individual, we will work closely with you and consult with you throughout the entire duration of that individual's care.
